^You can see the bulge in Google Streetview from two years ago. You can see that the mortar is a different color in the area, making me wonder if some work was done that was either an attempt to fix the problem, or possibly even a contributor to it.

Looks like water infiltration into the wall from the roof or parapet, then over time the saturated mortar disintegrates/erodes from the moisture itself and many cycles of freezing and thawing. Gravity does the rest.

Walked by it yesterday, the foundation is poured but no one was on site working. Hard to reconcile what they poured vs the renderings though, if the basement windows in the rendering match the foundation, the building will be sitting 4 ft above its neighbors and look very odd.

Does look that way. Hopefully the casting, brickwork and landscaping will bring it all together. There are others on the Fullerton blocks (4300-4400) of Westminster with somewhat similar elevations. The new construction single family home in the 4400 block of Westminster doesn't have a basement at all nor much of an elevation to the door, but turned out ok as set next to other homes with porch elevations, etc.

^The Park Place apartments will remain low-income and part of the Section 8 program.  The renovation will receive state and federal LIHTC, and "$35.5 million in tax-exempt multifamily housing revenue bonds".  All related LIHTC and Section 8 requirements and expiration dates will be extended post-renovation.

Whatever happened to it, I believe it's been gone for a few years now. I started walking around the neighborhood in 2017 when I worked in Cortex and only remember the foundation. I think someone turned it into a garden.

4308 Laclede? It looks like it was there in 2011. And then in Sept. 2014 the roof was missing and part of the back had been ripped off. So may have came down not long after that.
